For tests involving EtG, additional panels, or those exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale for precise measurement. If scalp hair isn't accessible, a comparable quantity of body hair can be used. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other types (including facial and axillary hair).

Process Overview

The drug testing laboratory process entails four fundamental phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, samples are initially processed into the lab system. This step involves checking the integrity of the sample’s seal and shipment, attributing a random LAN (Laboratory Accessioning Number), and completing any extra data entry requirements absent from an electronic chain of custody system.

Screening serves as a preliminary assessment for detecting drug abuse. While Screening offers a cost-effective method for negating drug use in many samples, court admission necessitates confirmation of any positive result. Any samples that presumptively test positive in Screening must undergo further confirmation.

In cases where a sample is presumed positive in Screening, additional hair is extracted from the initial sample for the Extraction phase. Here, drugs are isolated from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug testing the most complex methodology to execute.

The Confirmation of positive Screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to Confirmation, any presumptive positive samples are washed as necessary. From Accessioning through Confirmation, the comprehensive lab procedure is evaluated under CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
