A sample measuring 1.5 inches, consisting of approximately 200 hair strands (roughly the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample for screening and validation. For EtG, extra tests, and/or panels exceeding 10, a specimen weighing 150mg is preferred. We advise using a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, a comparable amount of body hair can be gathered. By head hair, we mean only scalp hair. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The four primary stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step involving the processing of a sample within a lab's workflow. This stage ensures the sample's proper sealing and shipping, the allocation of a unique LAN (Laboratory Accessioning Number), and the completion of any additional data entry absent from an electronic chain of custody system.

Screening serves as a preliminary rapid check for substances of abuse. Although Screening is an economical approach to exclude drug usage in most samples, a positive result in Screening necessitates confirmation to be valid in legal settings. Any samples with a presumptive positive in Screening need secondary confirmation.

If a specimen is identified as presumptive positive during Screening, additional hair is extracted from the original sample and readied for Extraction. In this stage, drugs are extracted from hair at a deeper concentration compared to other techniques (e.g., urine or oral fluid), which makes hair drug screening the most complex technique to execute.

Confirmation of any positive Screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The full lab procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
