For EtG, additional tests, or panels above 10, 150mg of the sample is advised. Utilizing a jeweler’s scale to weigh the sample is suggested. If scalp hair is not available, an equivalent amount of body hair can be used instead. "Head hair" references scalp hair solely. "Body hair" encompasses all other varieties such as facial and axillary hair.

Process Overview

The four essential stages in lab processing of drug test results are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of incorporating a sample into a laboratory's system. This involves checking that the sample was sealed and transported appropriately, designating a random LAN (Laboratory Accessioning Number), and finishing any additional data input not offered by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drug abuse. While this is a cost-efficient method to exclude drug usage for most samples, a positive outcome in screening must be verified to be legally valid. Samples showing presumptive positive results in Screening require subsequent verification.

Should a presumptive positive be found during Screening, more hair is retrieved from the original specimen to prepare for Extraction. During this step, drugs are extracted from hair at significantly lower concentrations compared to other methods like urine or oral fluid, contributing to the challenging nature of hair drug analysis.

Confirmation of any positive screening outcome employs GC/MS, GC/MS/MS, or LC/MS/MS methods. Samples with presumptive positives undergo washing before confirmation as necessary. The complete lab procedure from Accessioning to Confirmation is reviewed according to both the CAP (College of American Pathologists) Hair standards and the accreditation under ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
