Hair evaluation is increasingly recognized as an effective method for identifying drug and alcohol consumption. Hair retains extended histories of substance use by embedding biomarkers in the strands as they grow. When taken near the scalp, hair can reveal alcohol and drug use within an estimated 3-month timeframe. Hair is easy to gather, less susceptible to tampering, and straightforward to transport.
A hair sample of 1.5 inches (approximately the width of a #2 pencil) closest to the scalp, including about 200 hairs, yields 100mg, which is optimal for testing and validation. For EtG testing, add-ons, and screenings over 10 panels, a 150mg sample is advisable. Use a jeweler's scale to weigh the sample. If scalp hair isn't available, an equivalent amount of hair from elsewhere on the body can be used. For head hair, we specify scalp hair only. Body hair encompasses all other types (including facial and axillary hair).
Process Overview
The primary steps in lab processing of a drug screening result include Accessioning, Screening, Extraction, and Confirmation.
During Accessioning, a sample is first processed into a lab's system, which involves confirming proper sealing and shipment, assigning a LAN (Laboratory Accessioning Number), and inputting any extra data not covered by an electronic chain of custody system.
Screening is a preliminary check for drugs of abuse. It's a cost-efficient method to discount drug use in most samples, but a positive screen requires confirmation to be legally valid. Samples with preliminary positive findings in Screening need further confirmation.
If Screening indicates a possible positive, more hair is extracted from the initial sample for the Extraction phase. At this step, drugs are separated from hair at lower concentrations than other methods (e.g., urine or saliva), lending to the challenge of hair drug testing.
Confirmation of positive Screening results is conducted with GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples suspected to be positive are cleansed before confirmation as needed. The entire procedure from Accessioning to Confirmation is certified under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 standards.
