What it is, How it works

Hair analysis has emerged as an effective means for identifying drug and alcohol consumption. Hair serves as a long-term record of alcohol and drugs by embedding biomarkers within the hair strand’s fibers. Collected near the scalp, hair can offer a detection period of up to roughly three months for alcohol and drugs. Hair is easy to gather, somewhat resistant to tampering, and convenient to transport.

A 1.5-inch sample consisting of approximately 200 hair strands (around the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for screening and validation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is suggested. We advise weighing the sample on a jeweler’s scale. If scalp hair is not available, a corresponding amount of body hair can be collected. When mentioning head hair, we specify scalp hair only. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

The principal stages in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample into the lab’s system. This step verifies the sample’s proper sealing and shipment, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data not included in an electronic chain of custody system.

Screening is an initial swift assessment for drugs of abuse. While Screening is an economical approach to exclude drug use in most samples, a positive result must be validated to be court-admissible. Samples suspected to be positive in Screening need further confirmation.

If a sample is suspected as positive in Screening, more hair is taken from the initial sample for Extraction. During this phase, drugs are extracted from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to execute.

Confirmation of any positive screening result is conducted using GC/MS, GC/MS/MS or LC/MS/MS. All samples suspected as positive are cleansed before confirmation as needed. The complete lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
