A sample measuring 1.5 inches with approximately 200 hair strands (similar to the width of a #2 pencil) closest to the scalp offers 100mg of hair, the sample size ideal for both screening and confirmation. For EtG, additional tests, and/or those surpassing 10 panels, a specimen quantity of 150mg is advised. It's recommended to weigh the specimen using a jeweler’s scale. If scalp hair is not accessible, a comparable amount of body hair can be used. In discussing head hair, we exclusively mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

Laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial processing of a specimen into a lab’s system. This step includes verifying that the sample was adequately sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry not provided by an electronic chain of custody system.

Screening is an initial swift check for substances of abuse. Screening is an economical approach to rule out drug use in the majority of cases; however, a positive screen requires confirmation to hold up in court settings. Any samples that test presumptively positive during Screening necessitate secondary confirmation.

When a sample tests presumptively positive during Screening, further hair is extracted from the original specimen for Extraction. This step involves extracting drugs from hair at concentrations lower than in other testing methods (e.g., urine or oral fluid), highlighting why hair drug screening is the most intricate testing method.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All samples with presumptive positives are washed beforehand if needed. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair standard and adherence to ISO / IEC 17025 accreditation criteria.
