What it is, How it works

Recognized as a formidable method for identifying drug and alcohol consumption, hair testing captures a long-term record of substances by securing biomarkers in the fibers of a hair strand as it grows. When sourced near the scalp, hair can offer up to a 3-month detection period for alcohol and drugs. Hair collection is straightforward, challenging to tamper with, and easy to transport.

A 1.5-inch section of approximately 200 hair strands (equivalent to a #2 pencil) closest to the scalp yields 100mg of hair, the optimal amount for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a specimen of 150mg is advised. We suggest using a jeweler’s scale to assess the specimen weight. Should scalp hair be unavailable, a comparable quantity of body hair can be obtained. When mentioning head hair, we specifically mean scalp hair. Body hair pertains to all additional hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves integrating a sample into a laboratory’s system for initial processing. This encompasses verifying the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing supplementary data entry that isn’t covered by an electronic chain of custody system.

Screening entails a preliminary rapid check for drugs of abuse. Although Screening offers a cost-efficient means to exclude drug usage in the majority of samples, any positive indication must be verified to be permissible in legal proceedings. Samples showing presumptive positivity during Screening require a secondary validation.

Should a sample exhibit presumptive positivity during the Screening phase, additional hair is extracted from the original specimen for the Extraction process. In this stage, drugs are retrieved from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening notably complex to execute.

Verification of any positive screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positivity undergo necessary washing before confirmation. The entire laboratory sequence from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair designation and accreditation per ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
