For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the sample with a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be gathered. References to head hair specifically mean scalp hair. Body hair includes all other types (like facial, axillary, etc.).

Process Summary

The primary steps in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating a sample into a lab’s system. This ensures the sample was sealed and sent properly, offers a random LAN (Laboratory Accessioning Number), and completes any extra data entry not given by an electronic chain of custody system.

Screening is a quick initial evaluation for drugs of abuse. Though it is a cost-efficient approach to dismiss drug usage in most samples, a positive result requires confirmation for court admissibility. Samples that test presumptive positive in Screening need secondary confirmation.

If a sample tests presumptively positive in Screening, more hair is retrieved from the original specimen and prepared for Extraction. During Extraction, drugs are taken from hair at a lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug analysis poses more complexity in methodology.

Positive screening results need Confirmation through GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as necessary. The entire laboratory method from Accessioning to Confirmation is examined under CAP (College of American Pathologists) Hair criteria and aligned with ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
