When the sample is cut close to the scalp, it can reveal a detection span of up to roughly three months for alcohol and other substances. Hair collection is straightforward, relatively tamper-resistant, and simple to transport.

Collecting a 1.5-inch segment of around 200 hair strands (comparable to the width of a #2 pencil) nearest the scalp yields 100mg of hair, which is an ideal quantity for analysis and validation. For EtG, additional tests, and/or analyses exceeding 10 panels, it is advised to prepare 150mg of the hair specimen. We suggest measuring the sample with a jeweler’s scale. Should scalp hair be unavailable, a proportionate quantity of body hair can be acquired. By head hair, we mean scalp hair exclusively. Body hair encompasses all other types of hair (facial, axillary, and so forth).

Process Overview

The laboratory procedure for processing drug test samples involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing phase where a sample is incorporated into the laboratory’s system. This step confirms that the sample has been properly sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and concludes any additional data entry not covered by an electronic chain of custody framework.

Screening provides an initial swift assessment for the presence of drugs. It remains a cost-efficient approach to exclude drug usage in most samples, yet a positive finding in the screening phase necessitates confirmation for court validity. Any samples deemed tentatively positive during Screening mandate a follow-up confirmation.

For samples tentatively positive at the Screening phase, additional hair from the initial sample is readied for Extraction. Drugs are extracted during this stage from hair at much lower concentrations than in other techniques (such as urine or oral fluids), highlighting why hair drug assessments pose the greatest methodology challenges.

Verification of any tentative positive results is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples denoted as presumptively positive are cleansed before confirmation as required. From Accessioning to Confirmation, the entire laboratory workflow is scrutinized under both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
