For tests involving EtG, additional analyses, and/or panels beyond ten, 150mg of the sample is advised. It's advisable to measure the specimen using a jeweler’s scale. Should scalp hair be unavailable, an equivalent quantity of body hair may be used as an alternative. The term "head hair" exclusively refers to scalp hair. The term "body hair" pertains to all other hair types (including facial, axillary, etc.).

Process Overview

The four primary phases in the lab handling of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a lab's records. This step involves confirming the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and inputting additional data not captured by an electronic chain of custody system.

Screening conducts a preliminary rapid assessment for substance misuse. Although it serves as a cost-saving technique to exclude drug usage for most samples, a positive screening must be validated for legal admissibility. Samples that show presumptive positives during Screening require further confirmatory testing.

For samples showing presumptive positives during the Screening, additional hair is extracted from the original sample for Extraction. In this phase, drugs are isolated from hair at a considerably lower concentration compared to other methods (e.g., urine or oral fluid), highlighting why hair drug analysis is particularly challenging.

Verification of any positive screening output employs GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positives undergo a washing procedure prior to confirmation as needed. The complete lab process, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 certification.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
