A 1.5-inch segment comprising roughly 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the preferred sample size for conducting screenings and confirmations. For EtG, additional analyses, and/or assessments exceeding 10 panels, 150mg of the sample is advised. It is suggested to weigh the sample using a jeweler's scale. If scalp hair is not obtainable, a corresponding quantity of body hair can be acquired. In context to head hair, it specifically pertains to scalp hair only. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The primary stages in the laboratory analysis of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial processing of a specimen into the laboratory’s protocol. This process ensures that the specimen was correctly sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and completes any further data entries absent from the electronic chain of custody framework.

Screening provides an initial evaluation for substances of misuse. Despite Screening being an economical approach to detect drug usage in the majority of cases, a positive screening outcome must be substantiated to be valid in judicial situations. Any samples presenting as presumptively positive during Screening necessitate additional confirmation.

Should a sample test presumptively positive in Screening, additional hair is taken from the original collection and readied for Extraction. During this phase, drugs are extracted from hair at considerably lower concentrations compared to other techniques (e.g., urine or oral fluid), which is why hair drug screenings are the most complex methodologies to execute.

Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleansed prior to confirmation as required. The complete laboratory procedure from Accessioning to Confirmation is scrutinized following both the CAP (College of American Pathologists) Hair designation and the accreditation per ISO / IEC 17025 standards.
