What it is, How it works

When hair is gathered near the scalp, it can provide a detection period of up to about 3 months for alcohol and other substances. A sample measuring 1.5 inches consisting of around 200 hair strands (approximately the width of a #2 pencil) closest to the scalp will yield 100mg of hair, the optimal amount for screening and confirmation. For EtG, additional tests, and/or testing exceeding 10 panels, it is advised to use 150mg of the specimen. It is recommended to weigh the specimen with a jeweler’s scale. If head hair is not available, a corresponding quantity of body hair may be collected. When mentioning head hair, it specifically refers to scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory procedures necessary for processing a drug test result entail four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is processed into a laboratory's system. This phase includes verifying that the sample was properly sealed and shipped, assigning an arbitrary LAN (Laboratory Accessioning Number), and performing any supplemental data entry not provided by an electronic chain of custody system.

Screening facilitates a preliminary rapid examination for substances of abuse. Although Screening is a cost-efficient method to dismiss drug usage for most samples, a positive screen must be validated to be acceptable in court. Samples that show a presumptive positive in Screening are required to undergo secondary confirmation.

If a sample shows a presumptive positive during Screening, additional hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are retrieved from hair at a substantially reduced concentration than other approaches (e.g., urine or oral fluid), which explains why hair drug screening is the most challenging technique to execute.

Confirmation of any positive screening outcome is achieved using GC/MS, GC/MS/MS or LC/MS/MS. Before confirmation, all presumptive positive samples undergo washing as warranted. The complete laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
