What it is, How it works

The collection of hair is straightforward, relatively challenging to tamper with, and easy to transport.

A 1.5-inch section with roughly 200 hair strands (approximately the diameter of a #2 pencil) closest to the scalp yields 100mg, which is ideal for both initial tests and confirmations. For EtG, additional tests, and extensive panel tests exceeding 10, a 150mg sample is suggested. It's advisable to weigh the sample on a jeweler’s scale. If scalp hair isn't available, equivalent body hair can be used, where references to head hair mean only scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves entering a sample into the lab’s system. This process includes confirming the sample's proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any data entry missing from an electronic chain of custody.

Screening is a preliminary assessment for drug presence. Although it's a cost-effective method to exclude drug use in most samples, any positive result here must be confirmed for legal acceptability. Samples with positive presumption in Screening need further confirmation.

If Screening suggests a positive result, additional hair is retrieved for Extraction. Drugs are removed from the hair at much lower concentrations than other tests (e.g., urine or oral fluid), making hair drug screening the most complex method.

Confirmation of any positive Screening is done using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples presumed positive are cleaned before confirmation if needed. The full lab procedure, from Accessioning to Confirmation, follows the CAP (College of American Pathologists) Hair standard and ISO / IEC 17025 accreditation requirements.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
