For EtG, additional tests, or panels exceeding ten, a sample weighing 150mg is advised. We suggest utilizing a jeweler's scale for specimen weighing. If scalp hair is insufficient, a similar amount of body hair may be used. References to head hair pertain exclusively to scalp hair, while body hair includes all other types such as facial or axillary hair.

Process Overview

The laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a laboratory's system. This step entails confirming the proper sealing and shipment of the sample, assigning a random Laboratory Accessioning Number (LAN), and completing any supplementary data entry absent from an electronic chain of custody system.

Screening is an initial rapid evaluation for substances of abuse. Although Screening offers a cost-effective way to rule out drug usage in most samples, a positive screen requires confirmation to be admissible legally. All samples with presumptive positive results in the Screening need a follow-up confirmation.

In instances of presumptive positive results during Screening, additional hair from the original sample is prepared for the Extraction process. This phase extracts drugs from the hair at a significantly lower concentration relative to other methods (such as urine or oral fluid), making hair drug screening one of the most challenging methodologies to implement.

GC/MS, GC/MS/MS, or LC/MS/MS techniques are employed to verify any positive Screening results. All presumptively positive samples undergo washing prior to confirmation as necessary. The complete laboratory process from Accessioning through Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
