A sample measuring 1.5 inches of roughly 200 hair strands (about the width of a #2 pencil) close to the scalp produces 100mg of hair, which is ideal for analysis and verification. For EtG, additional screenings, and tests beyond 10 panels, a 150mg sample is advised. Using a jeweler’s scale to weigh the specimen is recommended. If scalp hair is not an option, a similar amount from other body hair can be retrieved. "Head hair" specifically denotes scalp hair, while "body hair" includes all other hair types (like facial, axillary, etc.).

Process Overview

The key steps in processing a drug test result in a laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is processed into the laboratory’s system, ensuring it was properly sealed and transported, assigning it a unique LAN (Laboratory Accessioning Number), and completing any additional manual data entry that might be absent from an electronic chain of custody system.

Screening is an initial procedure to identify potential drug use. Though a cost-effective screening can eliminate drug use for most samples, any positive screen must be confirmed for legal admissibility. Samples indicated positive in Screening require further confirmation.

Should a sample test positive during Screening, additional hair is extracted from the original sample to use in Extraction. Here, drugs are withdrawn from hair at much lower concentrations than other methods (like urine or oral fluids), making hair drug analysis particularly challenging.

Confirmation of positive screenings is achievable through GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicating a presumptive positive undergo a washing procedure prior to confirmation when appropriate. The entire sequence from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
