What it is, How it works

Hair testing has emerged as a formidable means for detecting drug and alcohol consumption. Hair encapsulates a prolonged history of alcohol and drug usage by embedding biomarkers within the fiber of the developing hair shaft. Collected near the scalp, hair can offer about a 90-day detection window for alcohol and drugs. Hair collection is straightforward, challenging to tamper with, and convenient to transport.

A sample of 1.5 inches consisting of roughly 200 hair strands (similar to the diameter of a #2 pencil), close to the scalp, will yield 100mg of hair, which is the optimal sample size for testing and verification. For EtG, additional tests, and/or screenings exceeding 10 panels, 150mg of the sample is advisable. We advocate using a jeweler’s scale for weight measurement. If scalp hair isn't accessible, a comparable amount of body hair can be collected. By head hair, we mean scalp hair exclusively. Body hair encompasses all other types of hair (facial, armpit, etc.).

Process Overview

The quartet of primary stages in laboratory processing of a drug test result includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the preliminary handling of a sample at a lab. This step involves ensuring the sample was properly sealed and submitted, allocating a random LAN (Laboratory Accessioning Number), and fulfilling any additional data input not provided by an electronic chain of custody system.

Screening features a swift initial check for substance abuse. While Screening provides a budget-friendly method to eliminate drug presence in most samples, a positive screen necessitates confirmation to stand in court. Any samples with presumptive positive results in Screening require further confirmation.

If a sample yields a presumptive positive during Screening, additional hair is taken from the original sample and prepared for Extraction. Here, drugs are retrieved from hair at concentrations significantly lower than other methods (such as urine or saliva), rendering hair drug testing as the most arduous methodology to execute.

Confirmation of any positive screening result is achieved using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positives are cleaned prior to confirmation, if needed. The complete laboratory course, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
