What it is, How it works

Hair analysis stands as an effective means for identifying drug and alcohol consumption. Hair records a long-term profile of substance intake by encasing markers within the strands as hair grows. When gathered near the scalp, hair presents a detection timeframe of up to around 90 days for substances. Collecting hair is straightforward, fairly hard to tamper with, and convenient to transport.

A 1.5-inch segment of about 200 hair strands (equivalent to a #2 pencil in size) nearest to the scalp provides 100mg of hair, the preferred quantity for analysis and substantiation. For tests like EtG, add-ons, or exceeding 10 panels, 150mg is advisable. We suggest weighing the sample with a jeweler’s scale. If scalp hair isn’t accessible, a similar volume of body hair can be taken. "Head hair" specifically means scalp hair, while "body hair" covers all other hair varieties (facial, axillary, etc.).

Process Overview

The drug test laboratory workflow includes four significant phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the sample's initial entry into the lab’s system. This phase confirms the sample’s proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any outstanding data entry absent from an electronic custody system.

Screening provides a preliminary rapid assessment for substance abuse. Although Screening efficiently excludes drug use for the majority of samples, a conclusive positive result requires verification to stand in legal settings. Samples showing presumptive positivity during Screening necessitate further confirmation.

If Screening suggests a positive, additional hair is extracted from the original sample to prepare it for Extraction. At this juncture, substances are extracted from hair at notably lower concentrations than in other approaches (e.g., urine or saliva), underscoring the complexity of hair drug analysis.

Verification of any presumptive positive during screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples flagged as potentially positive are cleansed before confirmation when needed. The entire procedure, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation coupled with accreditation to ISO / IEC 17025 benchmarks.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
