What it is, How it works

Hair captures long-term indicators of alcohol and various drugs by embedding biomarkers within the growing fibers of hair strands. When harvested near the scalp, hair can offer roughly a 3-month timeframe for detecting drugs and alcohol. Hair is easy to collect, relatively challenging to tamper with, and straightforward to transport.

A 1.5-inch segment consisting of about 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp provides around 100mg of hair, which is the optimal sample for testing and validation. For EtG, additional tests, or those exceeding 10 panels, 150mg of the sample is advised. We suggest utilizing a jeweler’s scale to weigh the specimen. If scalp hair cannot be acquired, a comparable quantity of body hair may be used. In context, head hair signifies scalp hair exclusively. Body hair includes every other hair type (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome consists of four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the primary handling of a specimen into a lab’s system. This involves confirming proper sealing and shipping of the sample, allocating a random LAN (Laboratory Accessioning Number), and performing any additional data entry that is not supported by an electronic chain of custody system.

Screening includes an initial rapid assessment for drug abuse. Despite being an economical method to negate drug use on the majority of specimens, a favorable screen requires a confirmation to be permissible in court. All samples that are potentially positive during Screening must undergo a subsequent confirmation.

If a specimen is potentially positive during the Screening phase, more hair is extracted from the original sample and readied for Extraction. At this step, drugs are isolated from hair at a concentration significantly lower than other methods (e.g., urine or oral fluid), underscoring the complexity of hair drug analysis.

Validation of any positive screening response is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all potentially positive samples are cleansed as needed. The complete lab process from Accessioning to Confirmation undergoes review under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
