What it is, How it works

When procured nearest to the scalp, hair can potentially reveal a history of substance consumption spanning up to roughly three months. Acquiring a 1.5-inch segment consisting of approximately 200 hair strands (roughly equivalent to the circumference of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal quantity for both screening and verification. For EtG enhancements, additional components, or extensive tests exceeding 10 panels, a 150mg sample is advisable. Measuring the sample on a jeweler’s scale is suggested. Should scalp hair be inaccessible, a commensurate quantity of body hair may be substituted. By head hair, we exclusively mean scalp hair; body hair refers to all remaining hair categories (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test is composed of four fundamental steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the initial phase where a specimen is integrated into the laboratory’s system. This encompasses ensuring the sample was adequately sealed and dispatched, allocating a unique LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry omitted by a digital chain of custody system.

Screening entails a preliminary swift assessment for substances of abuse. Although Screening is an economical method to dismiss drug usage in most samples, a positive screen requires confirmation to be court-admissible. Samples that test positive preliminarily during Screening necessitate further validation.

Should a sample show presumptive positivity during Screening, additional hair is extracted from the original sample and readied for Extraction. In this process, substances are extracted from hair at much lower concentrations than alternative methods (e.g., urine or oral liquid), highlighting the complexity of hair drug screening.

Confirmation of any positive result from Screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with preliminary positivity undergo washing before confirmation if necessary. The full laboratory process from Accessioning through Confirmation is scrutinized under both CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
