What it is, How it works

Hair testing has emerged as a robust method for identifying drug and alcohol consumption. Hair captures a longitudinal record of alcohol and drug intake by embedding biomarkers in the growing hair's fibers. When obtained near the scalp, hair can offer an up-to-3-month detection period for alcohol and other substances. Hair collection is straightforward, relatively resistant to tampering, and convenient to transport.

A sample measuring 1.5 inches, consisting of roughly 200 strands of hair (approximately the thickness of a #2 pencil) nearest the scalp, provides 100mg of hair, the perfect quantity for initial evaluation and verification. For EtG, additional tests, and/or assessments beyond 10 panels, 150mg of the sample is advised. We advocate for using a jeweler’s scale to measure the specimen. Should scalp hair be unavailable, a comparable amount of body hair can be collected. References to head hair specifically mean scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result is divided into four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a laboratory’s system. This process checks that the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry not covered by an electronic chain of custody system.

Screening provides a preliminary quick check for drug misuse. While screening offers a cost-efficient method to eliminate drug usage for most samples, a positive screen must be confirmed to be viable in court. Samples presumed positive in Screening demand a follow-up confirmation.

If a sample is presumed positive in the Screening phase, additional hair is extracted from the preliminary specimen and prepared for Extraction. In this step, drugs are extracted from hair at considerably lower concentrations than other methods (e.g., urine or oral fluid), which explains why hair drug testing is the most challenging method to execute.

Verification of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed before verification as required. The entire laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
