Collecting hair is straightforward, relatively hard to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 hairs (about the diameter of a #2 pencil) near the scalp yields 100mg of hair, which is ideal for assessment and validation. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is advised. The specimen should be weighed with a jeweler’s scale. If scalp hair is not an option, an equivalent amount of body hair can be used. When mentioning head hair, it is specific to scalp hair. Body hair covers all other varieties (facial, axillary, etc.).

Process Overview

The primary steps in a lab’s drug test processing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample into a lab's system. This process checks if the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry missing from an electronic chain of custody system.

Screening performs an initial swift screening for drugs of abuse. While Screening is an economical approach to eliminate drug usage on most samples, confirmation is needed for a positive result to be valid in court. Any samples indicated as positive in Screening require further confirmation.

When a sample tests as potentially positive during Screening, additional hair is extracted from the original sample for Extraction. During this phase, drugs are isolated from hair at a concentration notably lower than other methods (e.g., urine or saliva), which makes hair drug screening the most complex methodology to execute.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with potential positive results are washed before confirmation as necessary. The full lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
