What it is, How it works

Hair captures a prolonged record of alcohol and other drugs by embedding biomarkers within the strands of the hair as it grows. When gathered near the scalp, hair can reveal a detection window of up to nearly three months for alcohol and other drugs. Hair is straightforward to collect, somewhat resistant to tampering, and simple to transport.

A sample of 1.5 inches made up of around 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal quantity for assessment and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, it is advised to use 150mg of the sample. It is suggested to weigh the sample on a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair may be gathered. In the context of head hair, we mean scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The quartet of primary steps in the lab processing of a drug test result includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a lab’s system. This covers ensuring the sample was sealed and transported correctly, designating a random LAN (Laboratory Accessioning Number), and finalizing any additional data entry absent in an electronic chain of custody system.

Screening consists of an initial brisk appraisal for drugs of abuse. While Screening is an economical method to dismiss drug use in most samples, a positive result requires confirmation to be court-admissible. Any samples testing presumptively positive in Screening need a subsequent confirmation.

Should a sample appear presumptively positive during the Screening phase, additional hair is withdrawn from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at much lower levels than with alternate methods (e.g., urine or oral fluid), which is why hair drug testing is considered the most complex method.

Validation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation as needed. The complete lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 norms.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
