If scalp hair is unavailable, an equivalent quantity of body hair may be gathered. "Head hair" refers exclusively to scalp hair, while "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial sample processing into the laboratory’s system. This step includes confirming that the sample was adequately sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any required data entry not covered by an electronic chain of custody system.

Screening provides an initial, rapid assessment for drugs of abuse. While cost-effective for ruling out drug use in most samples, positive results from Screening necessitate confirmation for legal admissibility. Any samples showing presumptive positivity during Screening require subsequent confirmation.

Upon presumptive positivity in Screening, additional hair is sourced from the initial sample and readied for Extraction. At this stage, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug testing one of the most challenging methodologies to execute.

Confirmation for any positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples identified as presumptive positive undergo cleansing prior to confirmation, as required. The complete laboratory process, from Accessioning to Confirmation, is evaluated in accordance with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
