What it is, How it works

For EtG, additional tests, and/or panels exceeding 10, a sample of 150mg is advised. It's recommended to measure the sample using a jeweler's scale. If scalp hair isn't available, an equivalent amount of body hair can be collected. "Head hair" pertains only to scalp hair. "Body hair" includes all other types, such as facial, axillary, etc.

Process Overview

The laboratory processing of a drug test result encompasses four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a lab's system. This step includes confirming the sample was securely sealed and correctly shipped, allocating a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody.

Screening provides an initial swift check for drug abuse. Though a cost-efficient method to exclude drug use in most samples, a positive screen necessitates confirmation for legal credibility. Samples with presumptive positive results during Screening require secondary confirmation.

If a sample yields a presumptive positive during the Screening stage, more hair is extracted from the original specimen for the Extraction phase. Here, drugs are withdrawn from hair at a considerably lower concentration than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging to execute.

Confirmation of positive Screening results is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation as necessary. The entire lab procedure, from Accessioning to Confirmation, undergoes review under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
