Process Overview

The lab processing of a drug test outcome includes four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step in integrating a sample into a laboratory system, which involves checking that the sample's seal remains intact and that it was sent correctly, issuing a random LAN (Laboratory Accessioning Number), and entering any additional information not covered by an electronic chain of custody.

Screening serves as an initial rapid assessment to identify potential drugs of misuse. While screening is an economical means to exclude drug use in most cases, any found positives must be substantiated to be accepted legally. Samples that are tentatively positive during Screening require further confirmation.

If a sample tests tentatively positive during Screening, extra hair is extracted from the original sample for the Extraction phase. In Extraction, drugs are meticulously isolated from hair at substantially lower levels than alternative methods (like urine or oral fluid), presenting unique challenges in hair drug analysis.

Confirmation of any initial positive screening occurs via GC/MS, GC/MS/MS or LC/MS/MS. All provisionally positive specimens are cleansed prior to confirmation if necessary. The full lab procedure from Accessioning through to Confirmation is assessed under CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 accreditation.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
