What it is, How it works

Hair testing has become recognized as an effective method for identifying drug and alcohol use. Hair captures a prolonged history of substance use by embedding biomarkers within the fibers of the emerging hair strand. When obtained near the scalp, hair can offer up to a roughly 3-month detection period for drugs and alcohol. Hair is straightforward to collect, moderately challenging to adulterate, and convenient to ship.

A 1.5-inch segment of about 200 hair strands (roughly the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal quantity for analysis and corroboration. For EtG, supplemental tests, and/or panels exceeding 10, 150mg of the sample is advisable. We suggest weighing the specimen with a jeweler’s scale. If scalp hair is not present, a similar amount of body hair can be gathered. In the context of head hair, we refer specifically to scalp hair. Body hair encompasses other hair types (e.g., facial, axillary).

Process Overview

The four primary steps in the laboratory processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary handling of a sample into a lab’s system. This process ensures the sample was appropriately sealed and delivered, assigns an arbitrary LAN (Laboratory Accessioning Number), and completes any further data recording not available via an electronic chain of custody system.

Screening serves as an initial swift analysis for substances of abuse. While Screening is cost-efficient for eliminating drug use in numerous samples, a positive result requires confirmation for legal admissibility. All presumptive positive samples in Screening need a subsequent confirmation.

If a sample exhibits a presumptive positive outcome in the Screening phase, additional hair is extracted from the initial sample for Extraction. In this step, drugs are separated from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to conduct.

Verification of any positive screening result is achieved through GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Note: Although often described as "hair follicle tests", the test evaluates the hair strand, not the follicle beneath the scalp.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
