A hair sample measuring 1.5 inches, consisting of around 200 strands (comparable to the thickness of a #2 pencil), yields about 100mg of hair, which is optimal for both screening and verification. For EtG, supplementary checks, and/or tests that exceed 10 panels, a specimen of 150mg is advised. It is suggested to use a jeweler’s scale for weighing the specimen. If scalp hair is absent, an equivalent amount of body hair can be collected. The term head hair is specifically used for scalp hair, whereas body hair encompasses all other types (such as facial and axillary hair).

Process Overview

The laboratory handling of a drug test result is structured into four major stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial processing phase of a sample into a lab's system, which involves ensuring that the sample is properly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and completing any data entry not carried out by an electronic chain of custody arrangement.

Screening is a preliminary and prompt examination for illicit substances. Although it provides a cost-effective method to dismiss drug usage in most samples, a positive screening outcome must be confirmed for legal validity. Those samples that test positive in Screening require a follow-up confirmation test.

In instances where Screening suggests a positive result, additional hair is extracted from the initial sample for the Extraction phase. This stage involves the removal of drugs from hair at significantly lower concentrations than alternative methods (such as urine or oral fluids), which makes hair drug screening the most intricate procedure to conduct.

The Confirmation of any Screening results that show positivity is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Prior to Confirmation, all samples testing presumptively positive are cleansed as needed. From Accessioning through to Confirmation, the entire lab process adheres to the CAP (College of American Pathologists) Hair designation along with accreditation per ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
