A 1.5-inch segment containing roughly 200 hairs (similar to the thickness of a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the optimal sample size for analysis and validation. For EtG, supplementary tests, or panels exceeding 10, 150mg of the sample is advised. Using a jeweler's scale to weigh the sample is recommended. Should scalp hair be unavailable, a comparable quantity of body hair may be used. When we mention head hair, we refer specifically to scalp hair. Body hair includes all other hair types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result includes four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample within a lab’s system. It involves ensuring that the sample was properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry that may not be covered by an electronic chain of custody system.

Screening is a preliminary rapid test for illicit substances. Although Screening is a cost-efficient method to rule out drug use in most samples, a conclusive screen must be verified for legal use. Any samples that show a presumptive positive in Screening must undergo a secondary verification.

Should a sample show a presumptive positive during the Screening phase, additional hair is extracted from the original sample and conditioned for Extraction. This stage involves extracting drugs from hair at a concentration significantly lower than other methods (such as urine or oral fluid), highlighting the complexity of hair drug screening.

Verification of any positive Screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo cleansing as necessary before confirmation. The entire laboratory procedure, from Accessioning to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair designation and in line with ISO / IEC 17025 accreditation standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
