What it is, How it works

When hair is sampled near the scalp, it can offer approximately a 3-month timeframe for detecting alcohol and other substances. Hair is easy to collect, somewhat tamper-resistant, and simple to ship.

Acquiring a 1.5-inch segment of around 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is ideal for screening and validation. For EtG, additional tests, and/or panels over 10, 150mg of the specimen is advised. We suggest weighing the sample with a jeweler’s scale. If scalp hair isn't available, an equivalent volume of body hair can be utilized. Head hair refers solely to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary steps in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a sample into a lab's system, which entails ensuring the sample was correctly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not furnished by an electronic chain of custody system.

Screening offers an initial rapid assessment for substance abuse. As a cost-effective method, Screening can exclude substance use in the majority of cases; however, a positive screen must be verified for legal validity. Any samples with presumptive positive results during Screening require further confirmation.

For samples with presumptive positivity in Screening, additional hair is taken from the original sample and prepared for Extraction. During this phase, substances are extracted from hair at a considerably lower concentration than in alternative methodologies (e.g., urine or oral fluids), making hair drug analysis the most challenging methodology to conduct.

Confirmation of any positive screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo a wash before confirmation when necessary. The entire laboratory procedure from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair accreditation and the ISO / IEC 17025 standards.

Advantages of hair drug testing:

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
