For EtG, extended panels, and/or tests exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale for specimen weighing is suggested. If scalp hair isn't available, the same amount of body hair can be collected. Head hair refers solely to scalp hair. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

There are four primary steps in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with the preliminary handling of a sample into a lab's system. This step ensures the sample is properly sealed and sent, allocates a unique LAN (Laboratory Accessioning Number), and completes any necessary data not included by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of misuse. Although Screening is a budget-friendly method to rule out drug usage in most cases, a positive result must be verified for legal acceptability. Any samples showing a presumptive positive during Screening must undergo a secondary confirmation.

Should a sample show presumptive positivity during the Screening phase, more hair is drawn from the original sample and set up for Extraction. During this phase, drugs are obtained from hair at significantly reduced concentrations compared to other methods (like urine or oral fluid), making hair drug testing the most complex to conduct.

Confirmation of any positive Screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS technologies. Presumptively positive samples may be cleansed before confirmation if needed. The complete lab process, from Accessioning through Confirmation, is evaluated under both CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
