A sample measuring 1.5 inches and consisting of approximately 200 strands of hair (roughly the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the perfect quantity for analysis and validation. For EtG, supplemental testing, and/or screenings exceeding 10 panels, a 150mg specimen is advised. We suggest measuring the specimen's weight on a jeweler's scale. If scalp hair is absent, a similar quantity of body hair can be used. When referring to head hair, we imply solely scalp hair. Body hair encompasses all alternate hair types (facial, axillary, etc.).

Process Overview

The laboratory procedure for determining a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the introductory processing of a specimen into a laboratory’s setup. It ensures the sample's proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and finishes any additional data entry absent from an electronic chain of custody framework.

Screening is an initial rapid examination for drug abuse. Although Screening serves as a cost-efficient way to exclude drug usage in the majority of cases, a positive screen must be validated to hold up in court. Samples that presumptively test positive in Screening require subsequent confirmation.

In instances of a presumptively positive result in the Screening phase, further hair is extracted from the original specimen and readied for Extraction. Here, drugs are drawn from hair at a significantly lower concentration compared to other techniques (such as urine or oral fluid), which is a reason why hair drug screening is highly complex to execute.

Confirmation of any positive screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples shown to be presumptive positive are cleansed before confirmation as needed. The complete laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the standards of accreditation ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
