For EtG, add-on tests, or testing beyond 10-panel, 150mg of the specimen is advised. It is suggested to weigh the sample using a jeweler’s scale. If scalp hair isn't present, the equivalent amount of body hair can be used. By head hair, we mean only scalp hair. Body hair encompasses other types like facial or axillary hair.

Process Overview

The primary stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is entered into the lab system. This involves ensuring the sample is sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and filling in any missing data not captured by an electronic chain of custody.

Screening is a swift initial check for drug presence. Although Screening is an economical method to dismiss drug usage across most samples, a confirmed positive result is mandatory for court acceptability. Samples indicating presumptive positivity in Screening need additional confirmation.

When a sample shows potential positivity in Screening, extra hair is drawn from the original specimen for the Extraction phase. Here, drugs are isolated from hair at considerably lower concentrations than other methods (i.e., urine or oral fluids), making hair testing particularly challenging to conduct.

Confirmation for any positive result from Screening is done using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positives are washed before confirmation if necessary. The full laboratory process, from Accessioning to Confirmation, undergoes review per both the CAP (College of American Pathologists) Hair criteria and the standards of ISO / IEC 17025 accreditation.
