While highly accurate, hair tests may not detect recent drug use within the past 5 days, where urine tests remain superior.

What it is, How it works

Hair testing is acknowledged as an effective method for identifying drug and alcohol usage. Hair can retain a historical record of alcohol and drugs by locking biomarkers within the fibers of the developing hair strand. When cut from near the scalp, hair can offer up to an estimated 3-month period for the detection of alcohol and drugs. Hair is easy to gather, relatively hard to tamper with, and convenient to transport.

A 1.5-inch segment of about 200 hair strands (approximating the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for evaluation and validation. For EtG, additions, and/or tests exceeding 10 panels, 150mg of the collected material is advised. It is recommended that the sample be weighed using a jeweler’s scale. If scalp hair is missing, a similar quantity of body hair may be obtained. With head hair, we specifically indicate scalp hair only while body hair encompasses all other variations (such as facial, axillary, etc.).

Process Overview

The four primary phases involved in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a sample into a lab’s database. This process involves ensuring that the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and performing any additional data entry not covered by an electronic chain of custody system.

Screening denotes a preliminary rapid assessment for substances of abuse. Although Screening is an economical way to dismiss drug usage for the majority of samples, a screen yielding positive results needs confirmation for it to be permissible in legal proceedings. Any samples yielding preliminary positive results in Screening necessitate subsequent validation.

If a sample shows a preliminary positive result during Screening, more hair is extracted from the original specimen for Extraction. During Extraction, drugs are retrieved from hair at much lower concentrations compared to other techniques (like urine or oral fluid), which makes hair drug screening the most challenging method to conduct.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS or LC/MS/MS. All initial positive samples are cleansed before confirmation if required. The complete laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
