What it is, How it works

Hair captures a long-term record of alcohol and drug use by embedding biomarkers into the fibers of the hair as it grows. When hair is collected from near the scalp, it can offer a detection period for alcohol and drugs that lasts up to about three months. Hair samples are easy to collect, relatively hard to tamper with, and simple to transport.

A sample of 1.5 inches with around 200 strands of hair (comparable to the thickness of a #2 pencil) closest to the scalp will result in 100mg of hair, which is the ideal quantity for testing and confirmation. For EtG, additional testing, and/or panels exceeding 10, a specimen weighing 150mg is advised. We suggest using a jeweler’s scale for measuring the sample weight. If scalp hair is not available, an equivalent amount of body hair can be used. By head hair, we mean scalp hair exclusively. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step in processing a sample within a laboratory system. This phase involves confirming that the sample was adequately sealed and forwarded, allocating a random LAN (Laboratory Accessioning Number), and filling out any extra data entry missing from an electronic chain of custody system.

Screening constitutes a preliminary check for illicit substances. Although Screening is a cost-effective method for ruling out drug usage in most samples, a positive result must be confirmed to be valid in a legal setting. Any sample with a presumptively positive result in Screening requires additional confirmation.

If a sample indicates a presumptive positive result during Screening, more hair is extracted from the original specimen for the Extraction process. In this phase, drugs are isolated from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to carry out.

Positive screening outcomes are verified through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing as required before confirmation. The entire process, from Accessioning to Confirmation, is evaluated under both CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Novel Analytical Techniques: The Verona meeting witnessed the unveiling of cutting-edge analytical techniques, offering enhanced sensitivity, selectivity, and accuracy for drug testing.
