What it is, How it works

Hair testing is now widely recognized as an effective tool for detecting drug and alcohol use. Hair offers a long-term record of alcohol and other drugs by encasing biomarkers within the fibers of the growing strand. When gathered close to the scalp, hair can reveal a detection window of up to approximately three months for alcohol and other drugs. Hair is easy to gather, somewhat resistant to tampering, and simple to ship.

A 1.5-inch segment of roughly 200 strands of hair (around the width of a #2 pencil) closest to the scalp will provide 100mg of hair, which is the optimal sample for screening and confirmation. For EtG, add-ons, and/or screenings beyond 10 panels, 150mg of the specimen is advised. We suggest using a jeweler's scale for weighing the specimen. If scalp hair is not available, a similar quantity of body hair is acceptable. When discussing head hair, we are specifically referencing scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary stages involved in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample into a laboratory’s system. This process includes verifying the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not captured by an electronic chain of custody system.

Screening consists of an initial, rapid check for drugs of abuse. While Screening provides an economical means to rule out drug usage on most samples, a positive screen must be verified for court admissibility. Samples that are presumptively positive during Screening require a secondary confirmation.

If a sample is deemed presumptively positive during the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. In Extraction, drugs are removed from hair at significantly lower concentrations than in other methodologies (e.g., urine or oral fluid), which makes hair drug screening the most challenging to execute.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation if necessary. The entire laboratory procedure from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.
