What it is, How it works

Hair follicle analysis is recognized as an effective instrument for identifying drug and alcohol consumption. The hair strand offers a prolonged record of substance intake by embedding biomarkers within its fibers. When sourced proximate to the scalp, hair can indicate up to roughly a 3-month period of alcohol and drug usage. It's straightforward to collect, hard to tamper with, and simple to send.

A 1.5-inch segment of approximately 200 hair strands (around the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for evaluations and validation. For EtG, supplementary, or tests beyond 10 panels, 150mg of the material is advised. It is suggested to measure the material using a jeweler’s scale. If scalp hair is not present, a similar amount of body hair can be gathered. When discussing head hair, this specifically refers to scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four primary steps in the lab review of a drug test finding are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a specimen into a lab’s framework. This process includes confirming that the sample was properly sealed and sent, assigning an arbitrary LAN (Laboratory Accessioning Number), and fulfilling any extra data input not supplied by an electronic chain of custody system.

Screening is an introductory rapid inspection for drug abuse. Although Screening is a budget-friendly approach to discard drug use in most samples, a positive screen must be validated for it to be accepted in legal settings. Samples that show presumptive positives during Screening need an additional confirmation.

If a sample registers as presumptive positive during the Screening step, additional hair is obtained from the primary sample and is readied for Extraction. During Extraction, drugs are retrieved from the hair at lower concentrations as compared to other processes (ex. urine or oral fluid), which makes hair drug testing the most complex method to perform.

Confirmation of any positive screening outcome is undertaken via GC/MS, GC/MS/MS, or LC/MS/MS. Samples with presumptive positives are washed before confirmation if required. The comprehensive lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Remarkable advancements in sample preparation, detection, and identification of drug compounds have been presented. The manuscripts in this section presented novel approaches, such as multianalyte methods and on-line preparation which can enable rapid analysis and improve thorough forensic investigations.

Biomarkers and Pharmacokinetics: The identification and validation of reliable biomarkers and pharmacokinetic studies are fundamental aspects of drug testing and analysis. The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
