What it is, How it works

Hair analysis is now recognized as an effective method for detecting the use of drugs and alcohol. Hair can offer a comprehensive timeline of substance use by capturing biomarkers within the fibers of the hair as it grows. When taken near the scalp, this method can reveal drug and alcohol intake for up to about 90 days. Hair collection is straightforward, relatively resistant to tampering, and simple to transport.

A sample measuring 1.5 inches, consisting of around 200 hairs (roughly the thickness of a #2 pencil) closest to the scalp, will yield 100mg of hair, which is the preferred amount for initial testing and verification. For EtG, supplemental tests, and/or analyses exceeding 10 panels, 150mg of the sample is advisable. It's best to measure the sample's weight using a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be acquired. "Head hair" refers specifically to scalp hair, whereas "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The processing of a drug test result in a laboratory consists of four major steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes preliminary processing into the lab’s system, which includes checking that it was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and performing any supplementary data entry not covered by an electronic chain of custody system.

Screening serves as an initial expedited assessment for drug presence. It provides a cost-effective approach to rule out drug use on many samples; however, a positive screening requires confirmation for it to be admissible legally. Any samples deemed presumptively positive through Screening necessitate a subsequent confirmation.

Should a sample be presumptively positive at the Screening stage, additional hair is extracted from the primary specimen and set up for Extraction. Drugs are drawn out from the hair at a significantly diluted concentration compared to other techniques (such as urine or oral fluid), which accounts for the increased complexity of hair drug screening.

The Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive specimens are cleansed before confirmation as necessary. The entire lab process, from Accessioning to Confirmation, is evaluated according to CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
