A 1.5-inch cut of around 200 hair strands (approximating the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal quantity for both screening and confirmation. For EtG, additional tests, and/or panel tests exceeding 10, a specimen encompassing 150mg is advisable. We suggest weighing the sample using a jeweler's scale. If scalp hair isn't available, an equivalent portion of body hair may be utilized. By "head hair," we mean scalp hair exclusively. "Body hair" includes all other hair forms (facial, axillary, etc.).

Process Overview

The lab handling of a drug testing is divided into four primary phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial integration into a lab's system. This entails ensuring the sample's seal and shipment are intact, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening consists of an expedited check for substances of abuse. Though Screening is a budget-friendly choice to eliminate drug use in most samples, a positive result requires confirmation for legal acceptability. Specimens with a presumed positive in Screening necessitate further verification.

In scenarios of a presumed positive result at the Screening level, additional hair is extracted from the primary specimen for Extraction. This stage involves extracting drugs from hair, which occurs at lower concentrations than other methods (e.g., urine or oral fluid). This is why hair drug testing is considered the most complex method to execute.

Confirmation of any positive outcome following the screening is completed using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All presumed positive samples are washed when needed before confirmation. The complete laboratory workflow from Accessioning to Confirmation undergoes scrutiny per the CAP (College of American Pathologists) Hair designation and is aligned with ISO / IEC 17025 accreditation standards.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
