Process Overview

Accessioning, Screening, Extraction, and Confirmation are the four primary stages in the lab processing of a drug test result.

Accessioning involves the sample’s preliminary processing into a laboratory system. This entails confirming the sample was sealed and transported correctly, allocating a random LAN (Laboratory Accessioning Number), and finalizing any further data entry absent in an electronic chain of custody system.

Screening denotes an initial, swift examination for drugs of misuse. While Screening provides a cost-effective means of dismissing drug use for most samples, a positive screen necessitates confirmation for court acceptability. Samples presumptively positive in Screening require additional confirmation.

Should a sample be presumptively positive in the Screening phase, additional hair is taken from the original specimen for Extraction preparation. At this point, drugs are isolated from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), which renders hair drug screening among the most challenging techniques to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are cleansed as necessary. The entire lab process from Accessioning to Confirmation is conducted under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO/IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.
