A standard sample requires a 1.5-inch length and around 200 strands (approximate size of a #2 pencil) nearest to the scalp, yielding 100mg of hair, ideal for both screening and confirmation. For tests involving EtG, add-ons, and panels exceeding 10, a 150mg sample is advisable. We suggest using a jeweler’s scale for weighing. If scalp hair is unattainable, an equivalent portion of body hair can be used. Head hair refers specifically to scalp hair, while body hair encompasses all other types (like facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with entering a sample into the laboratory system, ensuring sample sealing and proper shipment, assigning a random LAN (Laboratory Accessioning Number), and completing incomplete data entry if the electronic chain of custody system lacks it.

Screening carries out an initial drug abuse check. While it is cost-efficient for excluding drug use in most samples, a positive result must be verified for court acceptance. Presumptive positive samples necessitate secondary confirmation.

If Screening shows a presumptive positive, additional hair is collected from the original sample for Extraction. Here, drug extraction occurs at lower concentrations compared to other methods (like urine or oral fluids), making hair drug screening a challenging process.

GC/MS, GC/MS/MS, or LC/MS/MS methods conduct the Confirmation of any positive screening result. Presumptive positive samples undergo washing before confirmation as necessary. From Accessioning to Confirmation, the complete laboratory process adheres to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
