A 1.5-inch section of about 200 hair strands (akin to the size of a #2 pencil) nearest the scalp yields 100mg of hair, ideal for both screening and validation. For EtG, supplemental, and/or assessments exceeding 10 panels, 150mg of the sample is advisable. It's suggested to measure the sample using a jeweler’s scale. Should scalp hair be unavailable, an equivalent amount of body hair is acceptable. Head hair specifically pertains to scalp hair. Body hair encompasses all other types (facial, underarm, etc.).

Process Overview

The laboratory processes a drug test result through four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial integration of a sample into a lab’s system. This step confirms that the sample was appropriately sealed and dispatched, assigns an arbitrary LAN (Laboratory Accessioning Number), and finalizes any further data entry not covered by an electronic custody chain system.

Screening involves an initial rapid assessment for substances of abuse. While Screening is an economical strategy to exclude drug use across most samples, a positive screen necessitates validation to be legally acceptable. Any samples indicating potential positivity in Screening require subsequent validation.

Should a sample show presumed positivity in the Screening phase, additional hair is extracted from the original sample for the Extraction phase. Here, drugs are isolated from hair at lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging technique to execute.

Validation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed before confirmation, as needed. The comprehensive lab process from Accessioning to Confirmation undergoes evaluation under the CAP (College of American Pathologists) Hair classification and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
