A sample measuring 1.5 inches with roughly 200 hair strands (similar in size to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for both screening and verification. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is ideal. We advise using a jeweler’s scale to weigh the sample. If scalp hair isn't accessible, a comparable quantity of body hair may be obtained. Head hair specifically implies scalp hair. Body hair encompasses all types of hair excluding scalp hair (such as facial, axillary, etc.).

Process Overview

The drug testing lab processes involve four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory step of processing a sample into a lab's system. This process includes checking if the sample has been properly sealed and dispatched, allocating a unique LAN (Laboratory Accessioning Number), and filling in any supplementary data gaps not covered by an electronic chain of custody.

Screening constitutes the preliminary drug abuse check. Despite being an economical way to dismiss drug occurrence in most samples, any positive initial read requires confirmation for courtroom admissibility. Presumptively positive screenings mandate further verification.

Once a sample tests presumptively positive during Screening, additional hair is retrieved from the original sample and prepped for Extraction. In this phase, drugs are reclaimed from hair at much lower concentrations than other methods (such as urine or saliva), making hair drug analysis one of the most intricate methods.

Verification of positive screening outcomes is performed by GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing before confirmation, as necessary. The entire process from Accessioning to Confirmation undergoes review in line with both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation criteria.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
