For EtG, additional tests, and those exceeding ten panels, 150mg of the specimen is advisable. It's suggested to weigh the specimen using a jeweler’s scale. If scalp hair isn't available, an equivalent mass of body hair may be gathered. Head hair pertains solely to scalp hair, while body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome encompasses four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial processing phase of a sample within a lab's system. This step involves confirming the sample was correctly sealed and transported, assigning a Random LAN (Laboratory Accessioning Number), and finalizing any remaining data input absent from an electronic chain of custody system.

Screening is an initial rapid assessment for drug abuse. Although Screening is an economical means to dismiss drug usage for most samples, a positive screen requires confirmation for court admissibility. Samples appearing presumptively positive during Screening necessitate secondary confirmation.

Should a sample test presumptively positive at the Screening stage, additional hair is retrieved from the original specimen and prepared for Extraction. During Extraction, drugs are withdrawn from hair at much lower concentrations than through other methodologies (such as urine or oral fluid), which accounts for why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples undergo washing as needed. The complete laboratory process, from Accessioning through Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
