For EtG, additional checks, and/or tests exceeding 10 panels, a sample of 150mg is advised. It's recommended to weigh the hair on a jeweler’s scale. If scalp hair isn’t available, a comparable quantity of body hair can be gathered. In context, head hair refers solely to scalp hair, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory processing of a sample within a laboratory's system. It includes verifying that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid assessment for substances of abuse. Although Screening is a cost-efficient method to eliminate drug use for most samples, a positive screen must be confirmed for it to be legally admissible. Any samples with presumptive positive results during Screening require subsequent confirmation.

Should a sample have a presumptive positive result in Screening, additional hair is extracted from the initial specimen and prepped for Extraction. In this phase, drugs are removed from hair at much lower concentrations compared to alternative methods (e.g., urine or oral fluid), which is why hair drug analysis is among the most challenging methodologies.

A positive result in Screening is confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples reflecting presumptive positives undergo washing before confirmation as necessary. The entire lab procedure, from Accessioning to Confirmation, is reviewed under the CAP (College of American Pathologists) Hair designation and is accredited to the ISO / IEC 17025 standards.
