What it is, How it works

Collecting hair is straightforward, it's not easily tampered with, and it ships without hassle.

A 1.5-inch portion containing about 200 hair strands (comparable in thickness to a #2 pencil) near the scalp yields 100mg of hair, optimal for both screening and validation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg is suggested. We advise using a jeweler’s scale for weight accuracy. If scalp hair cannot be sourced, the equivalent quantity of body hair might be obtained. By head hair, we specifically mean scalp hair. Body hair encompasses other types of hair (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test involves Accessioning, Screening, Extraction, and Confirmation as its primary stages.

In Accessioning, the sample is initially processed within the lab's system. This step includes ensuring the sample's seal and shipment were intact, assigning a unique LAN (Laboratory Accessioning Number), and filling in any missing details not captured by an electronic custody system.

Screening consists of a preliminary assessment for substance abuse. Although Screening provides an economical option to exclude drug usage in most cases, court-admissibility requires confirmation of positive results. Samples with a presumptive positive outcome in Screening demand an additional validation.

Whenever Screening indicates a presumptive positive, additional hair is utilized for Extraction. This process separates drugs from hair at a much lesser concentration than other methods (like urine or oral fluids), rendering hair drug screenings exceptionally challenging to execute.

Positive results from Screening undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Before Confirmation, presumptive positive samples are washed if necessary. The entire laboratory procedure, from Accessioning to Confirmation, is overseen per both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditations.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
