For EtG, supplementary tests, and/or examinations beyond 10 panels, 150mg of hair is suggested. Using a jeweler’s scale for specimen weighing is advisable. If scalp hair is unavailable, an equivalent amount of body hair is an alternative. References to head hair pertain strictly to scalp hair. All other hair types (including facial or axillary hair) are referred to as body hair.

Process Overview

The laboratory process for a drug test result incorporates four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is entered into a laboratory system. It includes verifying proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and filling in any additional data missing from an electronic chain of custody system.

Screening acts as a preliminary check for illicit substances. While it offers a cost-efficient method to exclude drug use in most samples, a positive screening must be validated to be court admissible. Samples that are presumptively positive during Screening necessitate subsequent confirmation.

If a sample indicates a presumptive positive in Screening, more hair is sourced from the original specimen for Extraction, where substances are abstracted from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), thus making hair drug analysis the most challenging approach.

Confirmation of positive screening outcomes is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positives undergo washing prior to confirmation, as necessary. The whole laboratory process spanning from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
