What it is, How it works

Collecting hair is straightforward, relatively resistant to tampering, and easy to transport.

A 1.5-inch portion of around 200 hair strands (comparable to the thickness of a #2 pencil) nearest the scalp yields 100mg of hair, perfect for both screening and confirmation. For tests involving EtG, add-ons, and/or over 10 panels, it's advisable to use 150mg of the sample. Using a jeweler’s scale for measurement is suggested. If scalp hair isn't available, a matching amount of body hair can be substituted. When discussing head hair, we mean exclusively scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processes a drug test result through four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the sample's initial integration into the lab's system. This step ensures the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any data entry not covered by an electronic chain of custody system.

Screening acts as a preliminary check for drug abuse. While it economically rules out drug presence in most samples, a positive screen must be confirmed for legal validity. Samples showing a presumptive positive result in Screening require further confirmation.

Should a sample yield a presumptive positive during Screening, additional hair is extracted from the initial sample for the Extraction phase. Here, drugs are retrieved from hair at a much lower concentration than in other methods (such as urine or oral fluid), making hair drug screening the most challenging to execute.

Confirmation of positive screening results is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed before confirmation if required. The comprehensive lab process—from Accessioning to Confirmation—undergoes review in line with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
