A sample measuring 1.5 inches, comprising approximately 200 hair strands (roughly the size of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional analyses, and/or tests beyond 10 panels, a quantity of 150mg is advised. Using a jeweler’s scale is suggested for weighing the specimen. If scalp hair is not available, an equivalent amount of body hair can be collected. The term head hair exclusively refers to scalp hair. In contrast, body hair includes all other hair types (facial, axillary, etc.).

Process Overview

Accessioning, Screening, Extraction, and Confirmation are the four primary steps in processing a laboratory drug test.

Accessioning refers to the initial logging of a sample into the lab’s system. This step includes confirming that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not handled by an electronic chain of custody system.

Screening is a preliminary quick assessment for illicit drugs. While Screening serves as a cost-effective method to rule out drug usage on most samples, a positive result necessitates confirmation to be considered legally valid. Any samples that exhibit a presumptive positive result in Screening require follow-up confirmation.

If a sample shows a presumptive positive during Screening, additional hair is collected from the original sample for Extraction. In this phase, drugs are extracted from hair at concentrations much lower than those used in other methods (e.g., urine or oral fluid), making hair drug testing the most challenging methodology to conduct.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positive screens undergo washing prior to confirmation as necessary. The comprehensive process from Accessioning to Confirmation is reviewed according to both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
