A 1.5-inch sample consisting of approximately 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample for both screening and verification. For EtG, additional assessments, and/or tests exceeding 10 panels, a specimen of 150mg is advisable. Utilizing a jeweler’s scale to measure the sample is suggested. In the absence of scalp hair, a comparable quantity of body hair can be collected. When mentioning head hair, we specifically refer to scalp hair. Body hair encompasses all other forms (facial, axillary, etc.).

Process Overview

The key stages in laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a laboratory’s framework. This process verifies that the sample was securely sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any further data entries absent from an electronic chain of custody system.

Screening includes a preliminary rapid assessment for substance abuse. Although Screening is an economical method to exclude drug use on the majority of samples, a positive result requires confirmation for court acceptability. Any samples that test presumptively positive in Screening necessitate additional confirmation.

Should a sample be presumptively positive at the Screening phase, additional hair is extracted from the initial sample and prepped for Extraction. During this stage, substances are extracted from hair at significantly lower concentrations than in other methodologies (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to conduct.

Confirmation for any positive screening outcome is undertaken through GC/MS, GC/MS/MS, or LC/MS/MS. All samples testing presumptively positive are cleansed prior to confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
