When collected near the scalp, hair can offer a detection window of up to approximately 3 months for alcohol and drugs. A 1.5-inch sample of around 200 strands of hair (similar to the thickness of a #2 pencil) nearest to the scalp yields 100mg, which is the perfect amount for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, a specimen of 150mg is suggested. We advise weighing the sample on a jeweler's scale. If scalp hair is not available, an equal quantity of body hair may be gathered. When discussing head hair, we specifically mean scalp hair. Body hair refers to any other types of hair (facial, axillary, etc.).

Process Overview

The laboratory testing of a sample for drug analysis includes four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the introductory processing of a sample into the laboratory's system. This process checks that the sample was appropriately sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody.

Screening is a rapid initial assessment for drugs of abuse. While it is a cost-efficient method for ruling out drug use in most samples, a positive result in Screening requires verification for legal acceptability. Samples with a positive indication in Screening require a follow-up confirmation.

If Screening shows a presumptive positive, additional hair is extracted from the original specimen for the Extraction stage. In this phase, drugs are isolated from hair at a much less concentrated level than other methods (such as urine or oral fluid), highlighting how challenging hair drug screening can be.

The Confirmation stage for any positive screening result is undertaken using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are cleansed as necessary. The entire laboratory examination, from Accessioning to Confirmation, is assessed under the CAP (College of American Pathologists) Hair guidelines and complies with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
